Each student must have their completed paperwork before stepping on the bus to see the provider. Please do not use PanCare Intake forms from previous years, as they have been updated.
Every required parent/guardian signature spot must be signed. The EL2 form has two sections requiring both the parent and student signatures (Pages 2 & 4).
If a student comes to the bus with any portion of the forms incomplete or missing a required parent/guardian signature, he/she may be asked to reschedule if appointments are still available. Students must have the original completed forms to present to the medical team on the bus.
Additionally, If copies are needed, the parent/guardian must complete the Authorization to Release Medical Information form (attached) and email it to MedicalRecords@PanCareFL.org. Once the completed authorization form is received, please allow up to 30 days for processing. Copies of requested forms will be sent via certified mail.


As we close out the Month of the Military Child, we wanted to salute our own students who either have families who serve, or plan to serve themselves. In addition, we spotlight our Master Sgt Hicks, along with 2002 Alum and former USMC fighter pilot, Aaron Bruner.

Save the Date!! VIP seats will go on sale Tuesday April 28th at 7:30am. Each student will receive 10 regular admission tickets with their cap and gown, but here is a great way to add additional seating for family and friends. Comfortable seats with backs! Payments can be made in the forms of cash, check, credit card (visa or mastercard), Apple pay, or Google pay.



Our military kids are true heroes in their own right, bravely navigating the challenges of service life alongside their families. On Purple Up Day, wear purple to honor their resilience, strength, and sacrifices.
Whether it's a purple shirt, a scarf, or even just a ribbon, let's paint the town purple!
