Thank you for all of the ways in which you continue to support your student(s) and our educators. This year has brought unique challenges for all of us but I am grateful for your continued understanding and partnership as we work through these unprecedented times together.
Bay County finds itself in an unenviable and unique situation. As a community we are dealing with back-to-back traumatic events which continue to impact everyone and especially our most vulnerable; the students of Bay District Schools. The stressors being placed on students, parents/guardians and staff members are affecting their ability to manage stress and anxiety. Our collective coping skills are being taxed and many of us need help. That’s why it’s my mission to provide both academic and socio-emotional support to our students so that they can thrive both emotionally and academically.
Attached to this memo, you will find a consent form which provides BDS the permissions necessary to screen your student for mental health stressors, to provide support to your student based on those screeners and to allow BDS to bill Medicaid (if applicable to your student) to offset the costs of those services.
The form is broken down as follows:
The first part requests parent/guardian consent for Bay District Schools to implement a socio-emotional screener to identify students who may be in need of additional support. Student WITH CONSENT will complete the screener and results will be confidentially shared only with staff members who will assist.
The next section provides consent for individual or group social-emotional support for students. If the screener indicates your student would benefit from additional support, this form provides written consent. Even though you may give consent on this form, YOU WILL BE CONTACTED PRIOR to any services being provided to your student.
The final section provides consent for Medicaid reimbursement. Billing for Medicaid reimbursement allows Bay District Schools to continue to fund our support services for students
